Edit a Care Plan

Let’s review how to edit a HEP Care Plan for a patient.

1. Use the Find a Patient search bar to locate the desired patient and select the Folder icon. This opens the Patient Overview page.

   

2. Click the Edit Care Plan icon. 

   

3. Drag and drop exercises in the Task List to reorder activities or to move them to a different phase.

   

4. Search for exercises and click Add to place the exercise on the Task List.

   

5. Remove any exercise by selecting the corresponding X.

   

6. Click Update Care Plan to save your changes. The patient will receive a notification that their Care Plan has been updated.
